Personal Health Assessment (PHA) through BCBS and WebMD

Please follow these steps to access your PHA online:

1. Go to: www.bcbsfl.com
2. At the top of the home page, click on My Blue Service Account
3. Go to: Member Resources
4. Click on: View Health Resources
5. Go to: Online tools to help you make informed decisions
6. Click on:signing on to My Blue Service
7. Set up your User ID (social security number) and your PIN (6 numbers)
8. Look under Personalized Health Tools from WebMD and click on More
9. Agree to the terms and conditions – click Submit
10. Click continue to by pass this page
11. Click continue to by pass this page
12. Next page, at the top where it reads: What is The Key to Improving Your Health? Take the Personal Health Assessment (PHA) – an online risk assessment from WebMD.
13. Click on: Personal Health Assessment (PHA)
14. Click (submit) in the section asking who is taking this survey.
15. Go ahead and take the Health Assessment online.
